The Kentucky MOST Coalition represents a state-wide stakeholder group promoting the “Medical Orders for Scope of Treatment,” approved as KRS 311.6225 in 2015 (amended in 2022.) We seek to transform how Kentucky approaches end-of-life medical decisions by providing greater education and awareness about the MOST Form and of advance care planning more broadly to providers and patients.

VISION
The Kentucky MOST Coalition promotes the education, acceptance, and implementation of the Kentucky “Medical Orders for Scope of Treatment” (MOST) form as a vital component of advance care planning for all Kentuckians.
Our vision is a Kentucky where people with advanced illness are equipped to make knowledgeable decisions regarding their end-of-life medical care and these decisions are known and respected across all healthcare settings.
